Step 1: Complete the Required Forms

A. Centrix DIA Form

This form is a FrankieOne document submitted via DocuSign.

You will need to:

• Provide basic information about your firm.
• Answer Question 3: Detail the nature of your firm’s business, including its core functions.
• Answer Question 4: Specify all purposes for which ID verifications will be conducted.
• Ensure your firm has the following policies and plans in place:
  • Privacy Policy
  • Information Security Policy
  • Risk Management Policy
  • Documented Plans:
    • Security Incident Plan
    • Business Continuity Plan

B. Centrix Application Form

This form is also a FrankieOne document submitted via DocuSign. It serves as the application for a subscriber account with Centrix.

Step 2: Approval Process
