Step 1: Complete the Required Forms
A. Centrix DIA Form
This form is a FrankieOne document submitted via DocuSign.

You will need to:
- Provide basic information about your firm.
- Answer Question 3: Detail the nature of your firm’s business, including its core functions.
- Answer Question 4: Specify all purposes for which ID verifications will be conducted.
- Ensure your firm has the following policies and plans in place:
- Privacy Policy
- Information Security Policy
- Risk Management Policy

B. Centrix Application Form
This form is also a FrankieOne document submitted via DocuSign. It serves as the application for a subscriber account with Centrix.

Step 2: Approval Process
After submitting both forms:
- DIA and Centrix will review and approve your application.
- Once approved, contact BGL to enable New Zealand passport verifications:
